PLEASE READ THE FOLLOWING STATEMENT:
I understand and agree to follow the rules of the application. I understand that if I am selected, payment is due by October 15, 2012. I will set up by 3:45 pm on Friday, November 30, 2012 and remain until the show closes at 4:00 pm on Saturday, December 1, 2012. I understand that Grace Place is not responsible in any way for loss, theft, or damage to my property or
injury to my person(s) as a result of this show. I also understand that I am responsible for the complete set up and maintenance of my booth. Additionally, I am responsible for collecting all sales taxes on items sold
and that Christmas in Berthoud and Grace Place makes no claims, guarantees, warranties, and/or other representations relating to quality, suitability, or safety of my products. My name below is my online signature that I agree with the rules of this application.

Indemnity Agreement

I (we) understand that participation in the Christmas Parade activities including but not limited to the parade, is accompanied by certain risks. In consideration for being allowed to participate in these activities, I (we) waive and release the Town of Berthoud, The Berthoud Area Chamber of Commerce, Fire Department, and other organizations and persons participating in this event from liability proximately caused by their negligence. I (we) agree to indemnify and defend all organizations and persons from liability arising from my (our) participation in the activities of the Christmas In Berthoud 2011 and the Parade of Lights. Entering the contact person's name is the signature of agreement to this form. (Must be entered by adult 18 or over, parent or guardian MUST sign for minor)

The parade is Saturday, December 1st, 2012.
Staging is at 4:45 PM and line up at 5:00 PM

Participants may begin to line up at 4:45 PM. The parade will stage at 2nd Street & Mountain Ave (lining up from south to north). The parade will go west on Mountain, turn north on 7th, circle Fickel Park to end on the east side of 6th Street for the Tree Lighting Ceremony.

The Christmas in Berthoud Team cannot guarantee Christmas gifts to all who request them. All nominees will be considered on the basis of need and funds available.

Please Note: You must contact the family and be sure they are willing to receive the gifts prior to submitting this form. If CIB is unable to accommodate the request, we will contact them. Families will be contacted prior to delivery.
